This form should take no longer than 10 minutes to complete.

To complete this form you will need:

• the names of the Council Tax payers
• your address and postcode
• your email address
• your Council Tax number
• details of any discounts or exemptions you are getting
• the details of the change in circumstances
• the date your circumstances changed

This form expires after 20 minutes of inactivity.
